Opal Kathleen "Kathy" Mullins, 84, of Ivydale, entered into rest Saturday, January 31, 2015.
Born May 18, 1930 in Clay, she was the daughter of the late Raymond and Pearl Samples.  In addition to her parents she was also preceded in death by her husband Ellis Mullins, brothers, Stanley and Guy Samples, sisters, Virginia Spencer, and Eileen Hanshaw.
Kathy is survived by her son, Ray Mullins, brother, George (Judy) Samples of Valley City, Oh, sisters, Ella (Tracy) Hanshaw of Seville, Oh, Ethel (Albert) Mullins of Lizemores and Pearl (Lynn) DeBoard of Apple Creek.
Funeral service will be held 2:00 P.M. Wednesday, February 4, 2015 at Wilson-Shamblin-Smith Funeral Home, Clay.  Burial will be in Williams Cemetery, Two Run.
Friends may call one hour prior to service.
